At Destination Specialists, we get plenty of inquiries about the passport application process. A few years ago, we even put up a blog post detailing the experience of one of our staff. That blog post has almost two hundred comments. Fortunately, the passport processing experience in the Philippines has gotten better, but it is still far from ideal.
That’s why we get plenty of inquiries with regards to processing passports. We figured we may as well post an FAQ on the subject.

What are the passport basic requirements for a first time applicant?
PSA/NSO Birth Certificate
PSA/NSO Marriage Certificate (for married female applicants)
Any two (2) of the following supporting documents:
- Land Title
- Seaman’s Book
- Elementary or High School Form 137 or Transcript of Records with readable dry seal
- Government Service Record
- NBI Clearance/Police Clearance
- Baptismal
- Voter’s Certificate, List of Voters and Voter’s Registration Record (for newly issued VRR, please attach receipt)
At least one (1) Valid Identification Card:
- Government-issued picture IDs such as the following:
- Digitized SSS ID
- Driver’s License
- GSIS ID
- PRC ID
- Voter’s ID
- UMID
- IBP
- Digitized BIR
- Senior ID
- Other acceptable picture IDs such as the following:
- College ID with Study Load
- Alumni ID
- Company ID with Employment Certificate
What are the requirements for First Time Application and Renewal of Passport of a Minor?
PSA/NSO Birth Certificate
PSA/NSO Marriage Contract of the child’s parents (if applicable)
Original Baptismal Certificate
Original Passport of the minor’s mother (for illegitimate children) or original passport of either parent (for legitimate children)
School ID, if the minor is a student
Original passport of the minor (if the application is or renewal)
What are the requirements for Passport Renewal?
Original Passport
Photocopy of the data page and back page of the passport
At least one valid Identification card
If your passport for renewal is colored green or has the XX passport number series, please submit:
- Original PSA/NSO Birth Certificate
- PSA/NSO Marriage Certificate for married women
- Supporting document listed above
What are the requirements for Lost Valid or Unexpired Passport?
All basic requirements for the First-time Applicant
Affidavit of Loss
Police Report
Clearing Period of fifteen (15) working days
Penalty charge of P200 (P350 for a lost electronic passport)
Copy of Passport that was lost, if one was kept
What are the requirements for Lost Expired Passport?
All basic requirements for a First –time Applicant
Affidavit of Loss
Copy of Passport that was lost, if one was kept
What are the documents required for First Time Adult Passport Applicants with Delayed Registered Birth?
1. Sequential or “Growing-up” photos
2. Additional forms of photo-identification (e.g., old driver’s license, old work ID, student card; old social club membership, etc.)
3. Documentary evidence to show use of name (e.g, pay slips; bank statement; college registration, etc; and
4. Evidence of registry at the application address (e.g., utility bills; official correspondence from public or private sector,etc.)
What are the requirements for Passport Applicants who reacquired Filipino citizenship under R.A 225 (Dual Citizenship)?
- Dual accomplished passport application form
- Personal Appearance
- Original NSO issued Birth Certificate
- Original NSO issued Marriage Certificate for married women
- Original Identification Certificate
- Original Oath of Allegiance
- Original Order of Approval for Reacquisition
- Original Certificate of Naturalization of Foreign Citizenship
